Niagara Re would be pleased to offer you a quote. Please use the following guidelines as minimum requirements for coverage:

• Minimum 25 employees (subject to state requirements)
• Minimum Specific deductible of $15,000
• Minimum 75% participation
• Maximum 15% commission
• RFP coversheet to include: company name, SIC code, effective date, Specific deductible(s) and contract basis requested, current Carrier and current rates (if available)
• Employee census (including name (or SS#) , DOB, sex, dependent status and zip code)
• 2-3 years large loss data (50% reports preferred)
• 2-3 years monthly paid claims data with corresponding enrollment
• Current Plan Document or SPD plus a listing of any proposed Plan changes
